Obituary of Mildred Foriska Kozak

Mount Pleasant, Mildred P. (Honey) Foriska Kozak, age 89 of Mount Pleasant, PA, died Saturday (08/25/12) in Excela Heath- Frick Hospital, Mt. Pleasant, PA. Arrangement have been entrusted to the Galone-Caruso Funeral Home, 204 Eagle St. Mt. Pleasant, PA Mrs. Kozak was born October 14, 1922 in Mount Pleasant, PA, the daughter of the late Matthew and Mary Dzurik Foriska. She was a lifelong member of Visitation of the Blessed Virgin Mary Roman Catholic Church of Mt. Pleasant, and its Christian Mothers and Rosary Altar Society. Mildred also enjoyed singing in the church choir and was a member of the First Catholic Slovak Ladies Association. She was a longtime employee of Kraisinger's Market in Mount Pleasant. Mildred is survived by her loving family: her children; Ronald M. Kozak and his wife Karen of Acworth, GA, John A. Kozak and his wife Dorothy of Mt. Pleasant, and Karen Yuhas and her husband Wayne of Economy, PA, and by her beloved grandchildren; Kevin Kozak and Sharlan, Michelle Kozak and her partner Angela, Jamie Kozak, Stacey Kozak and Gerrit, and Melissa Kaley and Jason, and by her five Great Grandchildren. In addition to her parents, Mildred was preceded in death by her husband, Albert J. Kozak in 1986, and her brothers and sisters: John, Joseph, Steve, Andrew, Michael, Emil, William and Alexander Foriska, Albina Forbes and Infant Mary Foriska. Family and friends will be received Tuesday from 2 until 9PM at the Galone-Caruso Funeral Home, 204 Eagle Street, Mount Pleasant. A blessing service will be held in the funeral home Wednesday at 12:30PM, followed by her 1:00 PM Funeral Mass in Visitation of the Blessed Virgin Mary RC Church, Mount Pleasant, with the Rev. Richard J. Kosisko as celebrant. Interment will be in the Visitation Parish Cemetery, Mt. Pleasant. A parish wake service will be held Tuesday at 3PM, and The Visitation Rosary Altar Society and Christian Mothers will pray the rosary Tuesday @3:15PM, both in the Funeral Home.
